Reg Tech Panel What makes systems and apps fail? More importantly perhaps, what makes them get back up again? Do you have faith in your organisation's application lifecycle management processes, or does IT operations have to slap expensive protective measures all over place just to ensure that the working day can be addressed with some degree of certainty?

It's an imperfect world, but short of getting a job in a kibbutz and pretending IT doesn't exist, are there ways of ensuring things don't go haywire without spending all your time and money watching them?

Here's a new survey you can really let rip on. We want to know about why systems fail and what can be done about it, including stuff like:

Is application resilience and recovery taken seriously enough? What are the most common causes of systems failure? What is the silliest cause of systems failure you have come across? What's your best "I told you so" story to do with systems falling over?
